Use of the Spirit of Giving Web site is at your own risk.
When using the Spirit of Giving Web site, information will be transmitted in an environment beyond the control of Mission Hospital. As a result, Mission Hospital assumes no liability for content posted by site users or any delay in removing content from the Spirit of Giving Web site. Content that violates any party will not be tolerated. Mission Hospital maintains the right to add to, modify or delete any posted materials on this Web site.
By submitting communications or content to the Spirit of Giving Web site, you agree that such submission is non-confidential for all purposes. Any failure to comply with all terms and conditions will result in a permanent termination of any posted material to the Spirit of Giving Web site.

Your privacy is very important to Mission Hospital. We do not sell or rent your personal information to third parties for their marketing purposes. Mission Hospital has taken measures to ensure that your personal information remains as secure as possible and in a trusted environment. We use encryption practices to help insure the integrity and privacy of the personal information and/or health-related personal information you provide to us. Although we make every reasonable effort to protect personal information and health-related personal information from loss, misuse, or alteration by third parties, you should be aware that there is always some risk involved in transmitting information via the Internet.

Clean Sweep Contest
NO PURCHASE NECESSARY. THIS DRAWING IS VOID WHERE PROHIBITED BY LAW.
To enter the sweepstakes, log on to www.spiritofgiving.org, fill out the “Post An Act of Giving” page, including email address, and submit your entry. Entries will be accepted via the Spirit of Giving website only from March 28, 2005-April 1, 2005. No entries will be accepted after 12 a.m. on April 1, 2005. All entries become property of Mission Hospital and its affiliates and will not be returned. Entry constitutes permission from contestant to release the entries, and all information contained therein, to Mission Hospital and its affiliates. Grand Prize drawings will take place on April 4, 2005. Grand Prize will be selected from all entries posted to the website within the time period of 3/28/05-4/1/05. Winner will be notified by email. Grand Prizes: (1) Organization by a Professional Organizer of 1- 2 rooms* of winners’ choice in winners’ homes. (Approx. value: $1500).
*Restrictions: Does not include garages, large storage areas that require several days of organization. Professional Organizer will be limited to spending a maximum of 6 hours at the winners’ homes. Prize may not be substituted. Prize must be claimed by 4/8/05 and Professional Organizer session must be booked for one day during the weeks of April 11, 2005 - April 22, 2005, between the hours of 9 a.m. and 6 p.m. Any prize not redeemed during the aforementioned dates will be void. All taxes are the responsibility of the winner. Entry constitutes permission (except where prohibited by law) to use winners' names, address and likeness for publicity purposes without additional compensation. Odds of winning are dependent on total number of entries received. Sweepstakes open only to residents of Orange County, CA. Participants release and agree to hold harmless Mission Hospital, St. Joseph Health System, the Ministry of the Sisters of St. Joseph of Orange, and their parents, affiliates, subsidiaries, advertising and promotion agencies and their respective directors, officers, employees, representatives and agents from any and all liability for any injuries, loss or damage of any kind to person, including death, and property, arising in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of the Grand Prize, participation in this sweepstakes, or participation in any sweepstakes-related activity. IN NO EVENT WILL SPONSOR, ST. JOSEPH HEALTH SYSTEM, THE MINISTRY OF THE SISTERS OF ST. JOSEPH OF ORANGE, OR THEIR PARENTS, AFFILIATES, SUBSIDIARIES AND RELATED COMPANIES, THEIR ADVERTISING OR PROMOTION AGENCIES OR THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES AND AGENTS, BE RESPONSIBLE OR LIABLE FOR ANY DAMAGES OR LOSSES OF ANY KIND, INCLUDING DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE DAMAGES ARISING OUT OF YOUR PARTICIPATION IN THIS PROMOTION.
Sponsor is not responsible for late, lost, illegible, misdirected, stolen, separated entries, or for any incorrect or inaccurate capture of information or the failure to capture such information. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the sweepstakes; to be acting in violation of the Official Rules; to be acting in a non-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. Sponsor reserves the right to terminate the promotion without notice.
For name of the Grand Prize winners mail a self-addressed stamped envelope for receipt by
April 30, 2005 to: Mission Hospital, Attn: Jason Koslow, 27700 Medical Center Road, Mission Viejo, CA 92691. The name of the winners will also be posted on spiritofgiving.org.
